(Prospective students may submit these forms electronically to intdec@bard.edu.)

Supplemental Forms

  • Noncustodial Parent's PROFILE
Whether the parents of the applicant are divorced, separated, or were never married, the noncustodial parent will be expected to complete the Noncustodial Parent's PROFILE after the student has completed the CSS PROFILE. This form is typically required when an applicant applies for financial aid for the first time.
  • Business/Farm Supplement Form (PDF)
If parents own a business or farm, this form may be requested by the Financial Aid Office prior to determining the student's financial aid package. This form is typically required when an applicant applies for financial aid for the first time.
  • PLUS Loan Request Form (PDF)
This form must be submitted to the Financial Aid Office if the parent of a dependent student wishes to apply for a Federal Direct PLUS Loan.

Verification Worksheets

Approximately 30 to 40 percent of those filing the FAFSA are selected by the federal processor to be verified; the Verification Worksheet must be submitted to confirm the size of the family's household and the number of students in college on at least a half-time basis. In addition, income must be verified for parent and student. The IRS Data Retrieval Tool on the FAFSA or the IRS Tax Return Transcript are acceptable forms of income verification. When a financial aid offer is made, the documents required for submission will be noted in the student's award letter.
