Hello, The following event may be of interest to you: Passport Fair Thursday, October 17, 2013 Get your passport renewed or get a new passport without having to leave campus!THE RED HOOK POST OFFICE IS COMING TO BARD TO PROVIDE A ONE-STOP PASSPORT SHOP FOR THE BARD COMMUNITY (from on-the-spot passport photos to the completion of the application process).PLEASE NOTE that specific documents are necessary for the process; these documents might not be at your immediate disposal, so read on! Mark your calendar and arrange NOW to get everything together so that you're prepared for the fair (the Thursday after we return from Fall Break).WHAT YOU'LL NEED TO BRING WITH YOU THAT DAY (PLEASE READ IN FULL):A. Passport Application Form DS-11 (UNSIGNED)You must use this form if any of the following apply to you: - You have never been issued a U.S. passport or - You are under age 16 or - You were under age 16 when your previous passport was issued or - Your most recent U.S. passport was issued more than 15 years ago or - Your most recent U.S. passport was lost or stolen or - Your name has changed since your previous U.S. passport was issued and you are unable to legally document your name change1. Blank forms will be available at the event2. You may access the form online and print it with your personal information entered (or print it out and fill it in by hand, but either way (DON'T SIGN IT UNTIL YOU'RE BEING PROCESSED!). Proof of US Citizenship (one of the following) - Previous US passport - Certified US birth certificate - Consular Report of Birth Abroad or Certification of Birth - Certificate of Naturalization or CitizenshipC. Proof of Identity (provide one of the following) - Certificate of Naturalization or Citizenship - Current, valid driver’s license - Government ID - Military ID - For minors under 14 years of age, each child must appear in person and both parents or legal guardians must present evidence of identityD. Two Identical Passport Photos ($15 on the spot at the fair!) - 2 x 2 inches in size - Taken within the last six months - Color preferred - Full-face, front view with a plain white or off-white background - Normal street attire, not hats, headgear or uniformsE. Payment for Passport Fees (Click here: http://travel.state.gov/passport/fees/fees_837.html for breakdown of fees and payment options. Passport photo services would be an additional $15, payable only by cash, check, or money order.)
