If your proposal has been approved, but you would like to make changes to your protocol, please complete the form below. Note: Amendments are reviewed on a rolling basis. If the IRB determines that your changes are minor, you will receive an updated approval letter. If the IRB determines that your changes are more than minor, you will be asked to submit a new proposal.
