Legally Change Your Name

  • Name Change Affidavit
    If you would like to register a legal change of name, please print and complete the Name Change Affidavit. The completed form must be accompanied by photocopies of the following documentation:
    1. A government-issued document with the new name.
    2. Proof of use of the former name.

    These documents can include:
    • Court order
    • Marriage certificate
    • Divorce certificate
    • Naturalization certificate
    • Passport
    • US Driver License
    • US State-Issued Identification Card
